OUR REFERRAL AND ASSESSMENT PROCESS

We welcome referrals from all Boroughs and we work closely with community Health Care Teams.

Referral and Admission Criteria

We admit male and female Service Users aged 18-65 with Forensic & enduring Mental Health needs, including Mild to Moderate Learning Disabilities, requiring short/long term supports. Service Users with Forensic history under Ministries of Justice (MOJ) and restrictions under the Mental Health Act 1983 (i.e. 37/41 and CTO) also benefit from our services.

Referral Route: -

Referral is usually accepted either directly from Community Health Care Team through their representative such as OT’S, Social Workers and Placement Officers.

Referral Criteria:

• An adult aged between 18 and 65 years • Forensic & enduring Mental Health needs including Mild to Moderate Learning Disabilities and Autism. Show commitment to engaging positively with the service

Assessment

A referral form is sent to the referrer, on return of the completed Referral form, an assessment date is agreed. An initial assessment of each individual will be done, to ensure that the services and facilities can meet the needs of the Service Users. Where suitable, an informal visit to the service could be arranged. This will enable the Service User/Advocate to make inform choices regarding the placement. Once the Service user is satisfied with the service arrangement further procedures will be followed accordingly, with respect to placement and funding. Individual can also make trial day visits prior to admission.

Admission

Admission of Service Users can be arranged throughout the day. Due to the structural facilities of the building, First Care Lodge Ltd cannot accept referrals from those with severe mobility difficulties.

Our team

The team at First Care Lodge Ltd is led by Registered Mental Health Nurses, Forensic Mental Health Professional and a qualified RMA with an array of experience in the field of Health and Social Care. Majority of our staff possesses either NVQ Level 2 or 3 or equivalent in Health & Social Care and we may endeavour to further their professional development.

Layout

All First Care Lodge Ltd premises are designed to provide supports in a ‘Family Type property. Service Users will benefit from spacious bedrooms, fully furnished with personal hand basins as well as fully fitted kitchens, two bathrooms/toilets, TV lounge/dining area and large gardens. One of the site’s in London Borough of Barking and Dagenham has en-suite facilities. Although First Care Lodge Ltd does not promote smoking, we acknowledge the needs of our Service Users and have provided a designated smoking area.

Accessibility and Facilities

First Care Lodge Ltd have care homes situated in the London Borough of Newham, London Borough of Barking & Dagenham, London Borough of Redbridge and The Royal London Borough of Greenwich.  All Homes are easily accessible by car and public transport with close links to buses and trains. It is also located closely to the North Circular Road (A406), A13 and M25 and The Blackwall Tunnel. First Care Lodge Ltd is close to our neighbouring Boroughs London Borough of Tower Hamlet and Havering. All Homes are close to many local amenities such as supermarkets, shopping centres, Post Office, banks, GP and Dental surgery, Religious establishments, Leisure Centres and Newsagents.

What will it cost?

We operate an individualised pricing system, which is based on the level of supports required by the Service Users.
